Background: Stroke is still widely reported as a major health burden. Poor control of hypertension and diabetes mellitus, often related to insufficient self-care behavior, places individuals at a higher risk of stroke. Objective: This study aimed to evaluate the effect of the transtheoretical model on self-care behavior among individuals at high risk of stroke. Method: A quasi-experimental approach using a pre-test and post-test in a single group was used in this study. Participants were 30 individuals with hypertension and diabetes mellitus who routinely attended Banjar Serasan Health Center for more than six months. Data on self-care behavior were collected using a validated questionnaire with a reliability coefficient of 0.870. Result: The median self-care behavior score before the intervention was 32 (IQR = 15), which reduced to 28 (IQR = 10) after the intervention. Wilcoxon signed-rank analysis indicated a statistically significant difference in self-care behavior score before and after the intervention (p = 0.002). Conclution: The transtheoretical model demonstrates potential as an effective approach to enhance self-care behavior among individuals at high-risk stroke groups in primary care settings.
