Abstract

Background: Difficult intubation is a major challenge in anesthesia that increases the risk of patient morbidity. Successful tracheal intubation is crucial for patient safety and the smooth conduct of surgical procedures. Difficult intubation may lead to serious complications, including hypoxemia, brain injury, and cardiac arrest. However, the accuracy of existing prediction methods remains variable, and no single method has been consistently reliable in predicting difficult intubation. Objective: This study aimed to analyze the correlation between the Neck Circumference-to-Thyromental Distance (NC/TMD) ratio and the degree of difficult intubation, as assessed using the Cormack–Lehane grading system, at Bekasi District Hospital. Methods: This analytical observational study with a cross-sectional design was conducted among adult patients undergoing elective general anesthesia between December 1 and December 31, 2025. The study included 47 patients scheduled for general anesthesia at Bekasi District Hospital. Anthropometric data were collected and analyzed using bivariate statistical analysis. Results: There was a statistically significant correlation (p < 0.05) between the Neck Circumference-to-Thyromental Distance (NC/TMD) ratio and the degree of difficult intubation based on the Cormack–Lehane grading system. Conclusion: A significant correlation was found between the Neck Circumference-to-Thyromental Distance (NC/TMD) ratio and the degree of difficult intubation, as assessed by the Cormack–Lehane grading system, in adult patients. These findings may strengthen preoperative airway assessment by anesthesiologists and serve as a basis for the development of airway management guidelines in Indonesia.
